Sports Physicals

Don’t drop the ball, get a sports physical at MyAlly Health. Sports physicals are required for participation in North Dakota and Minnesota student athletes. This is also known as a pre-participation exam. A medical provider will exam the student athlete to make sure that they are healthy enough to participate in sports. Sports physicals cost $40.

Please fill out and bring the following forms to your appointment:

• MN Sports Physical Form
• ND Sports Physical Form

At a sports physical the provider will:

• Check the patient’s basic health
• Look for cardiovascular and lung conditions
• Answer health care questions
• Evaluate the healing of old injuries

Vaccinations are offered at a sports physical, but may require parent’s permission.
